Need To Register With Us ?

We are currently accepting new patients.

Our catchment area is one mile radius of Tolworth (Toby Jug) roundabout on the A3. This includes Tolworth, Berrylands, Surbiton, parts of New and Old Malden.

We can only accept new patients who live in our practice area and are eligible for NHS treatment. When new patients register they must bring with them their medical card or complete a registration form obtained from the reception.  Proof of current address and right of abode in the UK must be produced: a recent utility bill and passport will be accepted.

Then, we will ask you to make an appointment to see the practice nurse for a new patient health check. Except in case of an emergency new  patients can see the doctor ONLY after this health check has been completed.

Change of Name, Address or Telephone Number

Please advise us with proof and in writing if you change name, address or telephone number. If you move outside the practice area you will usually need to register with a new surgery.

If you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct.

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.

Note that by sending the form you will be transmitting information about your self across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.

Alternatively you may print off a registration form, fill it out and bring it in with you on your first visit to the practice.
